Personal statement of self-certification

All JFCNP personnel are required to comply with the requirement for self-certification in their personal and official movements. If Italian law enforcement personnel request a Personal Statement of Self-certification, use the downloads below to assist in complying with their request. All JFCNP personnel and family members shall have a hard copy of the documents in their possession when travelling, including when traveling by foot. Remember that these documents do not authorize travel. Travel must still be in accordance with the requirements. The documents are a statement of a person’s reason for travel and the forms may be evaluated by the Italian authorities to determine if the travel complied with the travel restrictions. JFCNP personnel and family members are advised to comply with all directions of Italian law enforcement authorities, including if advised to stop traveling and to return home.
